Register with the Department of Health
On the Department of health login page, click the link under the “Patients” subheading called “Adult Patient Registration”
Enter Your Information
Enter information EXACTLY as it appears on the most recent form of your PA ID.
The Department of Health website is notoriously fussy about how information is entered, so if you receive an error message the first time, try again using a different format for your street address or unit. (Ex. instead of ST, use STREET)
After your registration has been accepted:
You will be prompted to create a username and password. It is important to save this information, as you will need to log into your account to make payments to the Department of Health and to check the status of your card.
When you have created your account, you will also receive a 6-digit Patient ID number. Write this number down.
